Overview

This survey is to provide feedback on the Enhanced-level training that you have just completed.  The feedback you provide will go directly to understanding the impact of the training sessions, and feed into improvements that will be made to the training as it is rolled out in the future.

All responses will be treated confidentially. Individual responses will not be identifiable; however, results will be grouped by local site to help us understand any location-specific insights. Your honest feedback is encouraged and appreciated.
